Insurance and Office Policy

Dr. Mangum and Aymi Bennhoff are not listed providers with any insurance company. Many insurance companies do, however, cover our services as out of network providers. If you need to call your insurance to verify coverage, please tell them the following:

                           You are coming in for a Comprehensive First Visit.

                           Dr. Mangum and Aymi Bennhoff are Non-Listed Providers

                           The billing code for the standard 90-minute visit is 99205.  

Dr Mangum and Aymi Bennhoff are not providers for MEDICARE or MEDICAID.  Your visit and possibly many of your tests will not be covered.  You cannot submit a “super bill” to them or to your supplemental insurance.

   Fees and Billing: Our payment policy is Fee-for Service.  We ask that you pay at the time of your visit. We will create a “Super Bill” which you can send to your insurance company or submit to your cafeteria plan for reimbursement. (if you lose your copies of the super bill there will be a charge for duplicates). In the event that full payment for charges incurred is not made, you will be responsible for all collection costs, court costs, and attorney’s fees. 

Possible additional costs may include:  Lab Fees and medications (which could be covered by your insurance plan).

Vitamins, supplements and/or herbs (can not be billed to insurance, but could be submitted to health spending or cafeteria plans).

   Appointments: Please be aware that Dr. Mangum or Aymi Bennhoff does not overbook or double book appointments;, therefore it is important that you are on time for your appointment and have your information completed.  We require a valid credit card to hold your appointment.

      Appointments cancelled or rescheduled without sufficient notice will be charged. 

                                                        “Sufficient Notice” is

                                                         24 hours or more  before your appointment

                                                          Less than 24 hours notice, 50% charge

                                                          No Shows and no notification will be billed at 100%

                                                         This applies to follow up and first time visits!

   Insurance Coverage:  Please remember that your insurance coverage is a contract between you and your insurance carrier.  If your insurance company requests additional information for reimbursement, you may be responsible for additional fees.

   Confidentiality:  Professional ethics as well as the laws of the State of Utah require that we honor your right to privacy and the confidentiality of our work together.  We will not provide information about you to others without your informed consent and written permission.  We are, however, required by law, to report clear and present danger to human life and any form of child abuse.

   Emergency Coverage: In case of emergency, you may leave a message with our answering machine, and we will return your call as soon as possible.  If the emergency is critical, or if you are unable to make contact with US personally, you should contact your local hospital emergency room, the crisis unit of your local Mental Health Center, or dial 911.
